Assessment Project
Background & Objectives
Rhondda Cynon Taf Borough Council understood that better outcomes for their citizens could be achieved through the introduction of assessment and longer term monitoring technology. Rhondda Cynon Taf commissioned Just Checking to conduct an assessment project for 269 individuals living in supported living settings.
Project Aims
The objective of the project was to develop responsive care models tailored to individual skills, desires and needs while effectively managing risk profiles.
- Review how support is utilised (day and night) and identify improvement areas
- Identify long-term digital technology enabled care (TEC) solutions that can enhance outcomes and contribute to delivering well-being principles
Just Checking worked alongside Rhondda Cynon Taf’s social work teams to deliver a personalised assessment package for each individual.
Outcomes
The outcomes of the project saw increased independence and autonomy for individuals, better use of council resources and a greater understanding of people’s needs. The assessment project has contributed to a more person-centred approach to care delivery, utilising technology to achieve better outcomes for people.
The Just Checking data has been key in providing evidence for any planned changes. The system gives us evidence of night-time activity and staff interactions in a very unobtrusive manner.
