Background
Cornwall Council has successfully embedded a sensor technology-enabled assessment process by utilising Just Checking’s full-time Managed Assessment Service for individuals requiring a care needs assessment.
Seeking new and innovative solutions to industry challenges, Cornwall Council embarked on a ‘test of change’ project with Just Checking in January 2023.
Managed Assessment Service
Just Checking provides activity monitoring services that help practitioners accurately assess support needs by providing objective evidence.
The Managed Assessment Service combines technology with high levels of hands-on support to help overcome the everyday barriers to embedding technology into care provision. This includes:
- A dedicated TEC specialist
- Technology provision
- Installation service
- Chart reading service
- Outcome tracking and review
- Consultancy and ongoing support
Outcomes and Statistics
The Council have utilised Just Checking’s Managed Assessment Service as a tech-first approach starting at the assessment stage, aiming to reduce, prevent and delay care escalations using TEC. The partnership has seen widespread positive outcomes:
- Keeping people at home for longer
- Enabling the least restrictive approach to support
- Evidencing health needs
- Signposting to complementary services
- Identifying TEC enabled solutions to meet needs and manage risk
- Giving people a voice and a platform to show their true capabilities
- Increase take up of TEC
